Get your YKHC medical records (MyYKHealth portal or Release of Information form)

Two ways to see your YKHC health information: (1) submit a Release of Information form to the YKHC medical records team, or (2) create a MyYKHealth account and use the HealtheLife app to view lab results, upcoming appointments, medications, immunization records, message your case manager, and - with consent - manage records for children under 18 or family members.
